1) Vaccines:
All animals must be up-to-date on the following vaccinations and tests.
a.
Dogs: Rabies, Distemper (DA2PPV/L), Bordetella, K9 Influenza
b.
Cats: Rabies, Distemper (FVRCP), negative for FIV/FelV
2) New Patients:
If this is the first time your pet has been to our facility, a New Patient Examination will be performed by the doctor at the owner’s expense.
3) Fecal:
Your pet must have a negative fecal test performed within the last 6 months along with being free of all external parasites (fleas, ticks, etc.) or they will be treated upon arrival at the owner’s expense.
4) Food:
If your pet will not eat the food provided, whether their own food or the kennel diet, we will accommodate them with a diet from our inventory at the owner’s expense.
5) Sedation:
If any form of sedation is required for treatment or handling of your pet to ensure the safety of our staff, the doctor has my permission to administer the medication they deem necessary.
6) Photos:
I give permission for Town & Country Veterinary Clinic to use my pet’s photograph and understand that no monetary benefits will be given.
